The Minnesota Department of Human Rights has notified (your company name) that our current Certificate of Compliance will soon expire.

 

Our company is a member in good standing of the Minnesota Mechanical Contractors Association (MMCA), which has an Affirmative Action Plan and Agreement until October, 2000 with the Minneapolis Department of Civil Rights that was approved by the Department of Human Rights in October, 2000 on behalf of all Association members.

 

(Include in the letter that you have either: (1) provided the Department with the required four semi-annual compliance reports required under the previous certificate, or (2) will provide the Department with the four semi-annual compliance reports prior to the expiration of your current certificate).

 

Based on this Agreement, it is our request that a new Certificate be issued for a two year term upon expiration of the current one.
